Intramyocardial hemorrhage contributes to microvascular obstruction in acute myocardial infarction

چکیده
Background Patients with hemorrhagic infarcts appear to constitute a high-risk group in acute myocardial infarction (AMI). However, the clinical implications of hemorrhagic versus non-hemorrhagic infarcts are currently unclear, warranting a more systematic and mechanistic approach towards understanding the underlying consequences. The question of whether hemorrhage is simply a bystander or contributes to additional myocardial injury remains to be investigated. The purpose of the study was to artificially induce hemorrhage in normal and infarcted (but not hemorrhagic) porcine myocardium to determine whether hemorrhage, per se, worsens prior ischemic damage.

Intramyocardial hemorrhage in spontaneously reperfused myocardial infarction.
OBJECTIVE. The presence of intramyocardial hemorrhage (IMH) is frequent in patients with acute myocardial infarction (AMI) undergoing primary percutaneous coronary intervention (PPCI). We aim for the presence IMH using cMRI in patients who presented AMI and did not undergo PPCI or thrombolysis. Cardiac magnetic resonance has proven to be a highly sensitive method for detect its presence in the ...

The goal of this systematic analysis is to provide a comprehensive review of the current cardiac magnetic resonance data on microvascular obstruction (MVO) and intramyocardial hemorrhage (IMH). Data related to the association of MVO and IMH in patients with acute myocardial infarction (MI) with left ventricular (LV) function, volumes, adverse LV remodeling, and major adverse cardiac events (MAC...
متن کاملHemorrhage contributes to chronic adverse remodeling in acute myocardial infarction: Insights from a novel preclinical model
Background Hemorrhage in association with microvascular obstruction (MVO) is a new independent predictor of adverse remodeling following acute myocardial infarction (AMI), occurring in ~35% of patients presenting with STEMI [1,2]. However, it remains unsettled whether hemorrhage is simply a marker of severity or directly contributes to the ongoing remodeling process. The aim of our study was to...
متن کاملCardiovascular magnetic resonance TE-averaged susceptibility weighted imaging of reperfused intramyocardial hemorrhage
Background In the setting of acute myocardial infarction (AMI), therapeutic and spontaneous reperfusion of ischemic myocardium can lead to interstitial intramyocardial hemorrhage (IMH) which is associated with microvascular obstruction (MVO) and subsequent adverse clinical outcomes. Imaging without contrast agents (native imaging) can be used in AMI patients for additional myocardial tissue cha...

Background Intramyocardial hemorrhage (IMH) secondary to myocardial infarction (MI) is associated with adverse prognosis [1]. The byproducts of blood breakdown lead to decreases in T2 and T2*, and have been shown to be related to microvascular obstruction (MVO) as detected by early contrast enhanced (EGE) imaging [2]. However, EGE is heavily influenced by contrast agent kinetics and measures th...
